Completion of this application does not activate a mailbox. A mailbox may be assigned only after required identification is verified and USPS Form 1583 is completed and approved.

To comply with federal regulations, two (2) government-issued forms of identification are required, including at least one photo ID. Identification will be reviewed during the verification meeting.

Acceptable forms of ID (select two) *

Unacceptable forms of ID include: HRA benefits cards, Social Security cards, birth certificates, credit cards, invoices, bills, and other similar items.

List any additional individuals or business names authorized to receive mail at this mailbox. (Only 1 additional person can be added. Additional USPS Form 1583 may be required.)
